From July 17 – 2021, individuals from Polish from abroad will have to complete an Electronic (Traveler Location Card – KLP) to replace existing paper cards. What is the Traveler Location Card (KLP)?

If you are crossing the border of the Republic of Poland by air after July 16, 2021, please fill out the Traveler Location Card before checking in.

It is designed for the safety of you, your loved ones, and everyone you come in contact with. If there is an infected person on the plane, the card will allow you and other passengers to access it quickly, which will help reduce the spread of the virus.

Purposes of Card:

When to use: Fill out the Card before the trip, during the travel, or at the latest before check-in.

Important! Filling out the form is mandatory when travel to Poland.

Who can benefit: Everyone who wants to travel by plane and cross the border of the Republic of Poland.

Requirements for Traveler Location Card:

You can need below the requirements to fill the KLP.

1 Country where the Travel began.
2 Flight Ticket number.
3 Date of arrival.
4 Applicant’s First name and Last name.
5 PESEL number or Applicant Identity Card (ID card or passport).
6 Applicant Contact details (Phone number and E-mail Address).
7 Addresses of all places you want to stay in Poland for the first 14 days (e.g., Family home, Hotel, Vacation accommodation).
8 Information about whether you have a certificate that you are recovered or fully vaccinated or whether you have a negative PCR / Antigen test for the SARS-CoV-2 virus.

How to apply for Travel Location Card?

It is quite simple to complete the request. Please follow the below steps carefully:

Step 1:

Click the link: https://aplikacje.gov.pl/app/klp/#/home

Step 2:

Enter the E-mail ID, and Click Confirm E-mail ID. And you will receive the mail with a link in your registered email id.

Step 3:

Click the link and complete the mandatory fields in the form (Details can be found in the section on what you need to prepare). Accept all declarations and consents.

Step 4:

Send your application. You will receive a confirmation letter to the E-mail address provided on the form. You can print the confirmation or deliver it to the airport staff on a mobile device (e.g., Mobile, Tablet).

Can a statement be submitted on behalf of someone else?

You can fill out a card on behalf of others traveling with you. You can provide multiple data on one card.

Information to Know: You can only fill out a card when multiple people are in the same places as you. For example, if you are traveling with your family but your children are staying elsewhere, you will need to apply for a separate form for them.

Additional information:

The data you provide on the Traveler Location Card is processed by (Sandepid – State Sanitary Inspection). The data administrator is the state poviat sanitary inspector who is eligible to travel or stay in a place where the traveler is subject to compulsory isolation or any other planned place of stay for travelers in the territory of the Republic of Poland.
